Once this form is completed, Our team will review your location to see if the territory is available and set up a call to learn more about your practice and share more about the Treehouse Eyes partner model.

Review and sign agreements
If your practice is a good fit for Treehouse Eyes we will review and sign the appropriate agreement for your state
3
Start helping more kids
Once an agreement is signed our Practice Development team will onboard you via Treehouse Eyes Academy, our proven training model. The entire team will then work with your practice to accelerate your growth as part of the Treehouse Eyes community of providers.
